NP Care ^TM is a physician directed, nurse practitioner driven nursing home, medical care support system. ECI has developed an EMR driven, patient risk stratification model, which helps direct our NPs and nursing homes to focus on patients at high risk for deterioration. We work within the facility with the medical staff to establish systems of care that drive nurse assessments, documentation and improved care.  The on-site NP is available to see residents with acute medical issues, review test results, evaluate wounds, start IVs, communicate with families and educate nursing staff.  NP Care ^TM improves the quality and consistency of medical care in nursing homes, preventing unnecessary ER and hospital transfers.

We do not replace the physician; however the physician often is not in the facility when the patient or a loved one is sick, so we are an extension of the physician,
KEEPING YOU COVERED. Our NPs are specially selected for their depth of medical knowledge, communication skills and ability to effectively collaborate with the nursing home team.

NP CARE^TM PROGRAM components

Our NPs are on site in Long term Care Facilities.

• Providing daily on site medical care that supplements attending physicians (minimal phone based medicine)
• We integrate care with the facility medical staff
• See residents when acutely ill
• Do risk assessments alerting family and facility to potential high risk medical events. (falls, stroke, etc.)
• We oversee discharge readiness, all patients are seen on discharge to assure safe transfer to community
• We see chronically ill residents
