Enhanced Care Initiatives (ECI) provides complex care
management services for medically unstable, complex patients
through a program of 24/7 clinical support and intensive
interventions that are based upon care plans guided by a
proprietary electronic health record (EHR) system. The
Company's services are offered through three
programs:
ECI is a new category in disease and complex care management.
Our programs focus on those with greatest medical needs
representing the highest costs,
leveraging state-of-the-art decision support software embedded
in an EHR with community or facility-based hands-on-care
and intervention. Our customers are
managed Medicare, Medicaid and specialty needs programs. Our
programs are driven by NPs, RNs, LPNs, social workers and physicians.
As a result of our approach, ECI rapidly improves health
status and decreases the need for hospitalizations, thus
controlling current and future costs.
